      "message": "[SCSI] libfc: Remove the reference to FCP packet from scsi_cmnd in case of error\n\nfc_queuecommand() allocates an FCP packet for each SCSI command and sends\nit out on the wire. In the process it stores the reference to the FCP packet\nin the scsi_cmnd structure.\nNow, in case under stress testing the libfc exchange layer runs out of\nexchanges the fc_queuecommand() may not be able to send out commands out on\nthe wire. In such a scenario if there is an error in sending the FCP packet\nout the wire; fc_queuecommand() deletes the FCP packet from internal queue,\nreleases the FCP packet and returns a SCSI_MLQUEUE_HOST_BUSY status to the\nscsi-ml.       "message": "[SCSI] pmcraid: reject negative request size\n\nThere\u0027s a code path in pmcraid that can be reached via device ioctl that\ncauses all sorts of ugliness, including heap corruption or triggering the\nOOM killer due to consecutive allocation of large numbers of pages.\n\nFirst, the user can call pmcraid_chr_ioctl(), with a type\nPMCRAID_PASSTHROUGH_IOCTL.  This calls through to\npmcraid_ioctl_passthrough().  Next, a pmcraid_passthrough_ioctl_buffer\nis copied in, and the request_size variable is set to\nbuffer-\u003eioarcb.data_transfer_length, which is an arbitrary 32-bit\nsigned value provided by the user.  If a negative value is provided\nhere, bad things can happen. When there is a change in vlan id, it does not have the capability\nto switch to different vlan interface. To solve this problem, a new structure\ncalled \u0027interface\u0027 has been introduced, and each hba can now have multiple\ninterfaces, one per vlan id.\n\nMost of the patch is a moving the interface specific fields from hba to the\ninterface structure, and appropriately modifying the dereferences.       "message": "[SCSI] scsi_lib: pause between error retries\n\nDuring cable pull tests on our 16G FC adapter, we are seeing errors,\ntypically reads to close targets, which fail due to CRC or framing\nerrors caused by the cable being pull (return status DID_ERROR).\nThe adapter detects the error on one of the first frames received,\nmarks the FC exchange as dead (further frames go to bit bucket) and\nsignals the host of the error. This action is so quick, and coupled\nwith fast host CPUs, creates a scenario in which the midlayer sees\nthe failure and retries the io almost immediately. We\u0027ve seen link\ntraces with the retry on the link while the original i/o is still\nbeing processed by the target. We\u0027re also seeing the time window\nfor the \"link to pull-apart\" and the physical interface to report\ndisconnected to be in the few millisecond range. Which means, we\u0027re\nencountering scenarios where the full retry count is exhausted\n(all with error) by the midlayer before the link disconnect state\nis detected.\n\nWe looked at 8G FC behavior and occasionally see the same behavior,\nbut as the link was slower, it rarely could exhaust all retries\nbefore the link reported disconnect.\n\nWhat is needed is a slight delay between io retries due to DID_ERROR\nto cover this error.  It is inappropriate to put this delay in the\ndriver, as the error is indistinguishable from other link-related errors,\nnor does the driver track whether the io is a retry or not. This is also\neasier than tracking between-io-error bursts that are seen in this\nscenario.\n\nThe patch below updates the retry path so that it inserts a delay as\nif the target was busy.  The busy delay is on the order of 6ms.       "message": "[SCSI] fix crash in scsi_dispatch_cmd()\n\nUSB surprise removal of sr is triggering an oops in\nscsi_dispatch_command().  What seems to be happening is that USB is\nhanging on to a queue reference until the last close of the upper\ndevice, so the crash is caused by surprise remove of a mounted CD\nfollowed by attempted unmount.\n\nThe problem is that USB doesn\u0027t issue its final commands as part of\nthe SCSI teardown path, but on last close when the block queue is long\ngone.  The long term fix is probably to make sr do the teardown in the\nsame way as sd (so remove all the lower bits on ejection, but keep the\nupper disk alive until last close of user space). There is no assignment for cpu from\nthat to the request_fn call to low level drivers.\n\nWhen this happens, the sc-\u003erequest-\u003ecpu will be using the init value of\n-1.  This will create a kernel panic when it hits bnx2i because the code\nrefers it to get the per_cpu variables ptr.\n\nThis change is to put in a guard against that and also for cases when\nbio affinity/queue completion to the same cpu is not enabled.
